-Let cashier know if you have already purchased a Spirit Card for your upperclassman so that the $25 discount can be applied to your Freshmen Spirit Card purchase.
ADDRESS INFORMATIONIf you move over the summer, please be sure to send an email to email@example.com with the students’ name, grade, and new address. Proof of residency will be required by providing a Utility Bill (Gas/Electric or Water only), Lease, House Contract or SSI or other government check. These items can be scanned and emailed or faxed to (502) 485-8770.
IMMUNIZATION POLICY / SOCIAL SECURITY NUMBER / BIRTH CERTIFICATEStudents whose immunizations expire and/or need a Td9 (adult tetanus and diphtheria—ages 14-16) during the school year 2016-2017 are required to have an updated ORIGINAL certificate at registration. The certificate must be signed by any duly licensed medical or osteopathic physician, nurse practitioner, or local health department provider; and must show dates all vaccines were administered. Students with an invalid certificate for the school year in grades 10-12 have been notified by mail. Please have your doctor date this certificate through your child’s year of graduation.
The Counseling Office will collect these ORIGINAL certificates during REGISTRATION.
Students are also required to have a copy of their Social Security Card and Birth Certificate as part of their school record. Please bring of copy of your child’s Social Security Card and Birth Certificate to Registration. These will also be collected by the Counseling Office.
GRAPHING CALCULATORSA graphing calculator is required for all math courses. The calculator that we recommend is the Texas Instruments TI-84Plus.
Students may already have one from middle school. We realize this is a substantial expense but feel that the graphing calculator is a valuable tool for learning mathematics Note: We do have a limited number of rental calculators available for the year. The rental fee is $40. Collection time/place will be announced in school. Rental agreement forms will be distributed the first day of school to be completed by the parent/guardian and must be returned before a rental calculator can be issued.
Additional supplies needed for math classes:
Graph paper—enough for everyday use (All Math Teachers) • Pencils—NO work accepted in pen • (Geometry) Compass from Male bookstore—this type is not available elsewhere—approx. $3.
PARENT VOLUNTEERS NEEDEDIf you would like to volunteer to work on one or more of the registration days, please call Nancy Caple, Male Volunteer Coordinator @ 502-608-5311. This is a great way to meet other parents and become familiar with Male and the staff.
SCHOOL SUPPLY LISTSStudents are free to choose any type of school supplies they wish. Make sure they come to school on the first day prepared with paper, pencil/pen, folder/binder. Teachers will tell students what is required for their class on the first day of school.

Volunteering at Male
We are so thankful for the parents at Male that have made the commitment to the traditional program. Part of that program is to foster community involvement and one of the best ways you can be a shining example of that spirit is to be a role model to your children. Please consider volunteering at Male as volunteer requests come to you through the email system. We have tried to make it as easy as possible to sign up through the use of SignUp Genius. We have plenty of opportunities in the evenings for those of you that work during the day.
We want you to be involved at Male and be a part of the Male Volunteer family.

Senior portrait REQUIREMENTS:
1. Dress- NO EXCEPTIONS!
2. The Male High School dress code as stated in the agenda must be followed. Dress code issues that are not allowed in your portrait are the following: inappropriate hair length and/or color (both girls and boys), any earrings other than post pearls (girls), nose rings (both girls and boys), facial hair (boys), and visible tattoos (both).
1. Type of pose: Head and Shoulder Shot – (Head size to chin: 1 inch maximum) No Dutch Shots
2. Overall yearbook photo size: 1 ¾ inches tall by 1 ½ inches wide, borderless
3. All yearbook prints: color with glossy finish
4. Plain Background- Dark Gray- In color. No color tinting, no outdoor shots
5. No photographer’s logo may appear on front of photo.
6. Digital photos are accepted. They must be 120% size at 300ppi. They can be submitted on CD, DVD, thumb drive/flash drive to the attention of Melanie Gossett at Louisville Male High School, 4409 Preston Highway, 40213.
